I - GUIDELINES FOR MISCELLANEOUS SERVICE IN THE INDIAN PASSPORT
- For all types of Miscellaneous Services, the "Miscellaneous Services Application Form" be duly filled up and signed. A passport size photograph to be affixed on the application.
- A copy (Notarized) of the valid Passport and visa/Green Card and Proof of Address be attached with every application.
- In addition to the prescribed fee, $ 20/- towards return mail charge be added to the fee if the application is submitted by mail. Applications be mailed by express/Fedex/DHL for expeditious delivery as well as for tracking purpose.
- For changes/correction of particulars in the laminated portion of the Passport i.e. correction of name, date/place of birth, change of address, spouse's name endorsement etc. it is advised to apply for new Passport in the 'New Passport Application Form'. (Fee of $ 40/-). A new passport valid for 10-year validity will be issued.
- There is no provision to change the name of father/mother once entered in the passport except (a) when a child is adopted under the Indian Adoption Law; or the father/mother of the applicant has changed their name after following the name change procedure.
- Inclusion of child's name in the parent's passport has been discontinued. A separate passport must be obtained by every individual.
- Emigration Check Not Required (ECNR) endorsement in Passport:
- Completed "Misc. Services" application form and one passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- A photocopy of academic degree/Higher Secondary Certificate issued by recognized educational institute in India and duly attested by the State Government/Ministry of HRD, GOI and the Consular Section of the Ministry of External Affairs, Patiala House, New Delhi.
- Fee of $ 10/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
- Change of Address in the Passport:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Proof of new address.
- Fee $ 10/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
- If the passport was not issued by the Embassy/Consulate where the application is submitted, this service will be provided after obtaining confirmation/clearance from the original Passport Issuing Authority (PIA) and it may take upto six weeks.
- Deletion of Child/Children's name endorsed in parent's passport/issue of deletion certificate:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Fee $ 10/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
NOTE: If one or both of the parents are abroad and application for the passport of the Minor is submitted to RPO in India by one parent or authorized person in India a consent letter/affidavit attested at the Embassy/Consulate to be submitted along with the application.
- Endorsement of Spouse Name in the Passport:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Marriage Certificate issued by the Registrar of Marriage and duly attested by the concerned State Government authority/Consular Section of the Ministry of External Affairs, Patiala House, Tilak Marg, New Delhi.
- Copy of the Spouse's valid passport.
- Fee $ 10/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
- Birth Certificate:
Birth Certificate should be obtained from the Registrar of Births under whose jurisdiction the birth occurred. However, for those who did not register the birth and require a birth certificate, may apply to the Embassy/Consulate for 'Certification of date of birth' as entered in the passport.
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Fee $ 20/-.
- Additional amount of $ 20/- to be added with the fee for return mail by
express/Fedex/DHL.
- Correction of date/place of birth:
As per existing instructions, date and place of birth once recorded in the Passport cannot be corrected unless the prescribed procedure is followed. The applicant should submit the duly corrected/amended birth certificate/school certificate on the basis of which the date/place of birth was first recorded in the first passport by the issuing authority or a Declaratory Order (not Sworn Affidavit before the Magistrate) from a First Class Judicial Magistrate regarding the correct date and place of birth of the applicant.

- Applicant's valid passport and a photocopy of pages containing
personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Amended copy of the Birth Certificate or a declaratory order (not a Sworn Affidavit attested by the Magistrate) of the First Class Judicial Magistrate regarding the correct date/place of birth.
- Fee $ 20/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
- Marriage Certificate i.e. Certification of entry in the Passport :
Marriage Certificate should be obtained from the Registrar of Marriage at the place of marriage/registration. The Embassy/Consulate may register and issue Marriage Certificate if the marriage was solemnized by the Consular/Marriage Officer at Embassy/Consulate under the Foreign Marriage Act of 1969 which requires advance notice in the prescribed form of at least one month, advertisement of notice of intention of marriage in newspapers at the place of residence and permanent residence under the provisions of the Act. However, in order to meet the requirement of Indian nationals who do not possess a Marriage Certificate issued by the competent authority, on application, the Embassy/Consulate may issue a document certifying the entry of marital status in the passport. (Please see the procedure for registration of marriage under the Act under section Consular attestation).

- Police Clearance Certificate (PCC):
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (first three and last two pages).
- A copy (Notarized) of valid visa/Green Card and proof of address.
- Fee $ 20/-.
- Additional amount of $ 20/- to be added with the fee for return mail by express/Fedex/DHL.
- If the passport was not issued by the Embassy/Consulate where the application is submitted, this service will be provided after obtaining confirmation/clearance from the original Passport Issuing Authority (PIA) and it may take upto six weeks.
II. Guidelines for Consular Attestation of Power of Attorney/Documents, including Academic Certificate, Birth/Death/Marriage Certificates, Driving Licenses and other documents:
- The attestation service is provided to Indian citizens. Documents submitted by other nationals will be attested only if the document is required to be submitted for a transaction in India or it originates from India. In the case of US nationals, their signatures should be first got attested by the Authentication Division in the State Department before submission of attestation at the Consulate. Other nationals should get their signature on the document attested first by their respective Embassy/Consulate. The Consular Officer reserves the right to accept or reject attestation of documents the contents of which are objectionable or contrary to the Rules.
- Application submitted at the Counter from 0900-1230 hrs. are serviced and returned from 1600-1700 hrs.
- Please insist on a receipt for the fee paid at the Counter and present the same for collection of the serviced document between 1600-1700 hrs.
- The fee is payable for each signature of the Consular Officer on the document.
- Fee once paid is not refunded after processing the application and the service is not approved.
- Fee is to be paid by Money Order/Cashier's Cheque. No personal cheque and/or credit card acceptable. Fee can be paid in cash at the Counter.
- Application submitted by mail will be serviced and returned within 7 working days.
- The Consulate takes no responsibility for postal delay or loss in transit of document/passport.
- Please add return mail charge of $ 20/- to the fee for return of the document by Express/Fedex/DHL mail to avoid delay/loss in transit.
- For all types of attestation services an application in the "Miscellaneous Service Form" duly filled in and signed by the applicant should be submitted along with a copy (Notarized) of first three and last two pages of passport containing personal particulars.
- Documents/Certificates originating in India should be authenticated by the Consular Section of the Ministry of External Affairs, Patiala House, Tilak Marg, New Delhi.
- Documents/Certificates originating from the USA should be authenticated by the US Department of State or concerned State Department of the State.
- Attestation of foreign public documents- The Hague Convention, to which India and USA are signatories, abolishes the requirement of legalization for Foreign Public Documents. With the certification by the Hague Convention ' Apostille', the document is entitled to recognition in India and no certification by the authentications office or legalization by Embassy of India or Consulates are required. Relevant provisions are contained at Article 1 of the ' Convention abolishing the requirement of legalization for foreign public documents' which is reproduced below:-
Article -I
The present convention shall apply to public documents which have been executed in the territory of one Contracting State and which have to be produced in the territory of another Contracting State.
For the purposes of the present Convention, the following are deemed to be public documents:
a) documents emanating from an authority or an official connected with the courts or tribunals of the state, including those emanating from an public prosecutor, a clerk of a court or a process-server('huissier de justice'):
b) administrative documents:
c) notarial acts:
d) official certificates which are placed on documents signed by persons in their private capacity, such as official certificates recording the registration of a document or the fact that it was in existence on a certain date and official and notarial authentications of signatures.
However, the present Convention shall not apply:
a) to documents executed by diplomatic or consular agents:
b) to administrative documents dealing directly with commercial or customs operations.

- Attestation of Power of Attorney/ Sworn Affidavit/Sponsorship Declaration:

b A photograph of the executant(s) to be affixed on the POA and notarized by the Notary Public.
c) An additional photocopy of all documents submitted for attestation be attached (stapled) to the application for office record. The original documents required to be attested and returned to the applicant should not be stapled to the application.

- The Executant(s) of the Affidavit/Power of Attorney can either sign the document before the Consular Officer or get the document notarized. The full name, particulars, photograph and signature of the applicant should correspond with the Passport.
- Fee: $ 20/- per signature for money/property matters; $ 10/- per signature for non-property matters.
- Additional amount of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
- Attestation of educational degrees/driving licenses/sponsorship certificate/True Copy Attestation::
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (i.e. first and last pages only).
- A copy of valid visa/Green Card and proof of address.
- The original document and a photocopy.
- Fee $ 10/-.
- Additional amount of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
Note: Documents submitted by foreign nationals will be attested only if the
document is required to be submitted for a transaction in India or it
originates from India. In the case of US nationals, their signatures should
be first got attested by the Authentication Division in the State Department
before submission of attestation at the Consulate. Other foreign nationals
should get their signature on the document attested first by their
respective Embassy/Consulate.
Documents/Certificates originating in India should be authenticated by the
Consular Section of the Ministry of External Affairs, New Delhi before
submission to the Consulate for attestation.
- Attestation of Commercial Documents (For attestation of Foreign Public Documents relating to commercial matters, please refer to the relevant section):
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a notarized photocopy of pages containing personal particulars (i.e. first and last pages only).
- A copy of valid visa/Green Card (in respect of non-US nationals) and proof of address.
- Document, in duplicate, duly attested by the concerned State Government authority.
- A letter of request along with copies of registration/license of the business firm.
- Fee $ 50/-.
- Additional amount of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
- Attestation of Adoption documents: (See guidelines for adoption at the website):
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Letter of Request from the adoption agency along with a copy of the valid license/registration.
- Two copies of the documents duly certified by the concerned Department of State.
- Fee $ 10/- per signature of the Consular Officer.
- Additional fee of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
- Registration of Death/Issue of Death Certificate and Issue of No Objection Certificate for Transporting body of the deceased/ashes to India:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Passport and a photocopy of pages containing personal particulars (i.e.
first and last pages only) of the deceased. Original Passport will be cancelled and returned.
- Death Certificate in original and a photocopy.
- Embalming Certificate (in original and a photocopy) stating that the body has been embalmed in accordance with the rules and regulations of international shipping and that the body has been placed in a hermitically sealed container with Zinc liner and wooden outer container.
- Certificate from the Department of Health stating that the deceased di not have any communicable/contagious diseases.
- Burial/Transit Permit.
- A copy of the flight schedule (confirmed or proposed) for transportation of the mortal remains.
- For transportation of Urn containing ashes of the deceased to India furnish Passport of deceased and Certificate from the Mortuary/Crematorium regarding contents of the Urn along with (i) & (ii) above.
- Any other relevant papers/investigations report on cause of death/employer's details etc.
- Fee: $ 1/- for Indian nationals and $ 20/- for other nationals.
- Additional fee of Mailing charge of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
- No Objection to Return to India (NORI) attestation and Waiver Certificate required by J1 visa holders. (Please see relevant section on the website www.indianembassy.org.
- Life Certificate:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (i.e. first three and last two pages only).
- A copy of valid visa/Green Card and proof of address.
- The applicant should appear in person and sign before the Consular Officer.
- If the applicant is unable to travel, applicant's signature be notarized and a Doctor's certificate submitted along with the application.
- Proof of pension eligibility, if for pension purpose..
- Fee for pension purposes - gratis. For other than pension purpose fee of $ 20/-.
- Additional fee of Mailing charge of $ 20/- to be added to the fee for return mail by express/Fedex/DHL.
- NRI Certificate:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a photocopy of pages containing
personal particulars (i.e. first and last pages only).
- A copy of valid visa/Green Card and proof of address.
- Proof of NRI status from the date from which the NRI Certificate is required. The old passports containing valid visas/Green Card will be sufficient. (Please attach photocopy of the old Passports).
- Persons of Indian Origin holding expired Indian Passport and valid foreign Passports may be issued with PIO status certificate and not NRI Certificate.
- Fee $ 20/-.

- Statement of Need/No Objection required by exchange scholars:
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- No Objection, in original, from Ministry of health to be furnished by the applicant (This is mandatory for seeking Statement of need for the first time)
- Notarized copy of the Contract with the Hospital/Institute on the standard Agreement Form with a letter from the Program Director on the official letter head with seal and contact telephone number.
- Written assurance in the prescribed form - available at Reception Desk.
- A copy of bio-data with address, telephone number, e-mail address/fax number
- Copy of passport (Notarized) showing personal particulars
- Proof of residence i.e. copy of utility bill/Driver License/Lease Deed etc.
- Fee $ 20/-.
- Additional fee of Mailing Charge of $ 20/- to be added to the fee for return mail by express/Fedex/DHL
- Renewal of No Objection Certificate:
- Notarized copy of the Contract with the Hospital/Institute on the standard Agreement Form with a letter from the Program Director on the official letter head with seal and contact telephone number.
- A copy of previous No Objection Certificate issued by the Consulate/GOI.
- Written assurance in the prescribed form
- Current IAP-66.
- A copy of the bio-data with address, telephone number and fax number/email address.
- Copy of passport showing personal particulars and validity of visa and passport.
- Fee $ 20/-.
- Additional fee of Mailing charge of $ 20/- to be added to the fee for return mail by express/Fedex/DHL
- Equivalency Certificate in respect of academic education.
- Completed "Misc. Services" application form and passport size photo affixed in the box.
- Applicant's valid passport and a notarized photocopy of pages containing personal particulars (i.e. first and last pages only).
- Notarized/certified copy of the Certificate and transcript. Original to be presented for verification and return.
- A letter from the Principal of the School/College regarding authentication of the document is required for attestation. Certificate issued by registered/recognized Institutes only will be attested.
- Fee $ 20/-. $ 20/- to be added for return mail by express/Fedex/DHL.
- Solemnization/Registration of marriage under Foreign Marriage Act, 1969 :
- At least one of the prospective bride or bridegroom must be an Indian national and a legal resident within the jurisdiction of the Embassy/Consulate.
- If either prospective bride or bridegroom is not an Indian national, a No Objection Certificate/Affidavit (in respect of individual regarding his/her marital status and willingness to marry an Indian) from the concerned Embassy.
- Three witnesses with their original valid passports.
- All the five persons are required to appear before Marriage Officer for signatures with their original passport and a photocopy both at the time of submission of form and at the time of solemnization of the marriage.
- After submission of Notice of intended marriage, newspaper advertisement is to be given by the individuals in local English newspaper as well as English newspaper in the place of permanent residence indicated in the Passport in India/concerned country. Text of advertisement is as under:

- Four identical passport size recent photographs of prospective bride and bridegroom.
- New notice when marriage not solemnized within six months. Whenever a marriage is not solemnized within six months from the date on which notice thereof has been given to the Marriage Officer, the notice and all the proceedings arising therefrom shall be deemed to have lapsed and fresh notice is to be given.
